Larian Studios has just released a brand new Baldur's Gate 3 Hotfix, addressing various issues, bugs, and problems in the game. As always, some changes were applied to the story and companions. In this hotfix, Karlach and Wyll seem to be the ones receiving the most new content and attention from the developers. If you wish to check out the full patch notes for hotfix16, you can do so here. Let's take a look at all the changes related to BG3 companions in the latest hotfix:
Karlach Changes in Hotfix 16
- Karlach will now receive her scene in Avernus, regardless of whether she is an avatar or companion.
- Resolved the issue where avatar Karlach (Origin) could initiate the initial romance scene with Lae'zel after proving her worth, which should not be possible due to her engine not being upgraded yet.
- Wyll will follow her to Avernus correctly.
Wyll Changes in Hotfix 16
- Wyll will now follow Karlach to Avernus correctly
- If Wyll doesn't have a partner and he's the Blade of Avernus, he will appear during the initial Epilogue scene.
- Wyll is now appropriately celebratory upon the Netherbrain's defeat.
- Wyll now has a line during the ending scene when realising the tadpoles are gone
Gale Changes in Hotfix 16
- Fixed Gale's dialogue ending abruptly when asking him for a kiss.
- Gale is less likely to lie to you about wanting to return the Crown of Karsus when making his decision in the endgame.

Astarion Changes in Hotfix 16
- Fixed Astarion not being available through the companion menu at camp if you sent him directly to camp during the recruitment dialogue.
Lae'Zel Changes in Hotfix 16
- If you're playing Origin Karlach you won't be able to initiate the first romance scene with Lae'zel after proving your worth to her, due to Karlach's engine still not being upgraded.
- Tweaked the trigger conditions for Lae'zel's initial romance dialogue after proving your worth to her so that it only triggers if the player controlling her has a single avatar. This is to avoid accidentally triggering the dialogue with an unintended avatar in case you have multiple.
Shadowheart Changes in Hotfix 16
- New dialogue option when talking to Shadowheart in the Epilogue, fixing a path that only gave you one choice.
